We are currently taking applications for mobile food vendors at our Music on Main music series events.

Mobile Food Unit License Application

The Village of Capac Code of Ordnances requires that all mobile food vendors obtain a Village license to operate on both private and public lands.

Application fees have been waived for the Music on Main events. Simply fill out the application form to submit your vendor request.

Insurance Coverage and Permits (Proof Required)

a) Proof of General Comprehensive Liability policy with limits of no less than $1 million Combined Single Limit Coverage issued by an insurer licensed to do business in the State of Michigan and which names the Village as an additional injured. b) Proof of Public Liability and Property Damage motor vehicle policy with limits of no less than $1 million issued by an insurer licensed to do business in the State of Michigan. c) Provide copies of all licenses and permits, including, food preparation/service permits issued by the Department of Health.

Hold Harmless/Indemnification

To the fullest extent permitted by law the undersigned individually, and on behalf of the applicant (if a business), shall defend, pay on behalf of, indemnify, and hold harmless Village of Capac, its elected and appointed officials, employees and volunteers, and others working on behalf of the Village of Capac against any and all claims, demands, fees (including attorney fees), injuries (including death), suits, property damages, or loss, including all costs connected therewith, and for any damages which may be asserted, claimed, or recovered against or from the Village of Capac, arising in any way out of or involving the operation or use of a Mobile Food Unit in the Village of Capac.

Acknowledgement

I have read and agree to the above hold harmless/indemnification clause as well as the Mobile Food Unit Ordinance and Mobile Food Unit Regulations and agree to abide by the same. I understand that Village grounds and property will be left in a clean and neat condition after use and I agree to pay for any damage to the grounds/property which may incur as a result, in any way, out of or involving the operation or use of a Mobile Food Unit in the Village of Capac.
